Job summary

Enhance Rehabilitation is seeking qualified Physical Therapist candidates to join our team in a SNF setting. You will evaluate and treat patients following an individualized plan of care.

You will collaborate with facility staff, supervise Physical Therapist Assistants, re-evaluate progress, and maintain comprehensive documentation while communicating effectively with patients and families.

Responsibilities

• Evaluating new patients, developing plans of care and setting measurable, functional goals
• Treating patients and educating them on their injury, progress and probable outcomes
• Collaborating with facility staff to ensure excellent patient care
• Supervising Physical Therapist Assistants
• Re-evaluating patient progress and updating plans of care
• Completing timely documentation including evaluations, re-evaluations, discharges and daily progress notes
