You will supervise Physical Therapist Assistants, re-evaluate progress, and ensure thorough documentation of evaluations, progress notes, and discharges.

Responsibilities

• Evaluating new patients and developing plans of care.
• Setting measurable, functional goals with patients.
• Providing excellent care to patients in SNF.
• Delivering outstanding customer service to patients and families.
• Collaborating with facility staff to ensure quality care.
• Supervising Physical Therapist Assistants.
• Re-evaluating patient progress and revising plans of care.
• Documenting evaluations, re-evaluations, discharges, and daily notes.
