Job summary

Enhance Rehabilitation is seeking qualified Physical Therapists to join our team in a SNF setting. You will evaluate and treat patients following an individualized plan of care, while providing excellent patient and family service.

You will collaborate with facility staff to ensure high-quality care, supervise PTAs, and complete timely documentation. This role emphasizes measurable goals, patient education, and progress re-evaluations to optimize outcomes.

Qualifications

• Treating patients, answering their concerns and educating them on their injury, progress and probable outcomes
• Supervising Physical Therapist Assistants
• Re-evaluating patient progress, revising plans of care and resetting goals
• Complete timely and accurate documentation including; evaluations, re-evaluations discharges and daily progress notes.
