Job summary

Qualifications

• Bachelor's Degree in Physical Therapy required.
• BLS certification within 6 months.
• Wisconsin Physical Therapist license required.

Responsibilities

• Interprets physician referrals and completes musculoskeletal, neuromuscular, cardiopulmonary, and/or integumentary evaluations utilizing appropriate methods.
• Establishes a plan of care and implements individual treatment based on evaluation results and functional goals.
• Monitors and evaluates outcomes and adjusts the treatment plan; provides status updates to physicians.
• Completes patient discharge planning including home programs and family teaching; documents progress and manages billing and scheduling.
