Qualifications

• Licensed Physical Therapist (IDFPR)
• BLS certification within 30 days of hire (AHA)
• Bachelor’s Degree in Physical Therapy (Required)

Responsibilities

• Demonstrates ability to perform patient transfers with assistance as needed from bed, chair and floor.
• Uses safe and appropriate body mechanics to reduce risk of injury to self, staff and patients.
• Designs an appropriate plan of care that integrates goals, treatment, outcomes, and discharge plan.
• Educates patients, family, caregivers, students, and other health care providers using relevant teaching methods.
• Evaluates clinical findings to determine physical therapy diagnoses and outcomes of care.
• Performs examination and selects reliable examination methods relevant to chief complaint, screening, and history.
• Produces documentation in accordance with the CTSCP67 policy and EMR systems.
• Screens patients to determine effectiveness and need for further therapy services.
