Qualifications

• Must hold a Bachelor's Degree in Physical Therapy.
• Licensed Physical Therapist (PT) in good standing (IDFPR).
• Basic Life Support (BLS) certification within 30 days of hire.
• Commitment to ongoing continuing education for acute stroke care.

Responsibilities

• Demonstrates ability to perform patient transfers with assistance as needed from bed, chair and floor, using safe body mechanics.
• Educates patients, family, caregivers, students, and other health care providers on treatment plans and progress.
• Designs an appropriate plan of care that integrates goals, treatment, outcomes, and discharge plans; adjusts care based on patient response.
• Evaluates clinical findings to determine physical therapy diagnoses and outcomes of care.
• Performs examinations with reliable methods relevant to chief complaint and history.
• Produces documentation in accordance with CTSCP67 policy.
• Screens patients to determine need for therapy services and effectiveness of care.
