• Bachelor’s degree in Physical Therapy from an accredited program.
• Current state Physical Therapy license and ongoing CE requirements.
• Proficiency in Net Health and Point Click Care software solutions.

Responsabilités

• Evaluate residents to determine PT needs and rehabilitation potential.
• Develop and update individualized treatment plans to improve mobility and independence.
• Supervise PT assistants and coordinate with nursing staff and families on progress.
• Document timely and accurately per facility and payer requirements.
• Educate residents and caregivers on home exercise and safety strategies.
