Covenant Living and CovenantCare at Home in California are seeking a dedicated physical therapist to provide comprehensive therapy services within the home health setting. You will evaluate patients, develop and implement treatment plans, and collaborate with families and care teams to ensure optimal recovery and mobility.

Qualifications include a Bachelor's in Physical Therapy with an active PT license and CPR certification; a Master’s in Physical Therapy is preferred.

Qualifications

• Bachelor’s degree in Physical Therapy from an accredited program.
• Current PT license and CPR certification, per state requirements.
• Experience: 2+ years in physical therapy or related setting.

Responsabilités

• Assess therapeutic, rehabilitative, functional status and plan care.
• Evaluate home environment and capabilities.
• Assess muscle strength, mobility, ROM and rehab potential.
• Direct and supervise physical therapy treatments.
• Instruct patients, families, and caregivers on therapy goals and exercises.
• Participate in discharge planning and care conferences.
• Provide home health PT consultation and inservice education.
