Job summary

Covenant HealthCare is hiring a Physical Therapist to provide homebound therapy, develop and adapt therapy programs, and educate patients and caregivers. The role emphasizes mobility-focused rehabilitation and collaboration with the care team.

The PT will assess, plan, and implement treatment, teach patients to use equipment, and participate in team meetings to ensure high-quality, patient-centered care. A valid Michigan PT license is required.

Qualifications

• Graduate of an accredited school of physical therapy with Michigan license.
• At least one year of experience as a Physical Therapist; homecare experience preferred.
• BLS certified or eligible.

Responsibilities

• Evaluate patients using diagnostic/prognostic muscle, nerve, joint and functional assessments.
• Discuss discharge goals and provide home exercise program prior to discharge.
• Treat patients to relieve pain and restore function using exercises, gait and transfer training.
