Job summary

Well Care Health in North Carolina seeks a licensed Physical Therapist for home health. You’ll assess, plan, and implement patient-focused care across adolescents to geriatrics, with a 40-hour week and variable shifts, driving required.

You must hold a PT degree from an accredited program, NC/SC license and CPR, with at least 1 year of experience and strong communication, safety, and documentation skills. Some weekends/holidays may be needed.

Qualifications

• Graduate of Physical Therapy program from an accredited university.
• Licensure in NC/SC and CPR certification.
• Minimum 1 year of experience.
• Knowledge of home health and physical therapy procedures; ability to use modalities and assistive devices.
• Ability to transfer and guard patients safely.
• Strong interpersonal skills with patients, families, and physicians.

Responsibilities

• Assesses, interprets, plans, implements, and evaluates patients.
• Admits patients to the agency when PT is a primary skilled need.
• Assists with program development and discipline-specific operations.
• Communicates and documents information per agency policies.
• Contributes to program effectiveness.
• Maintains and adjusts schedule to enhance agency performance.
