Job summary

Good Shepherd Rehabilitation seeks a licensed Occupational Therapist to restore function and prevent disability for patients with rehabilitation needs. The role involves assessing patients, planning and implementing therapy, and collaborating with the care team to meet treatment goals.

You will document progress, adjust care plans, and provide education to patients and families while maintaining a safe, infection-controlled environment on a treatment unit.

Qualifications

• Assessing patient condition and response to treatment.
• Collaborating with health-care team, patients and families.
• Documenting interventions and adjusting plans as needed.
• Maintaining infection-control practices and patient confidentiality.

Responsibilities

• Assess patient condition upon order of physician and at regular intervals during shift.
• Integrate patient information to facilitate holistic patient care.
• Set goals, collaborate and communicate with health-care team, patients and families.
• Act on physician’s order and patient need in a timely manner.
• Document interventions and responses; revise care plans based on patient response.
• Monitor delivery of care and identify improvements; intervene to maintain plan of care.
