Job summary

Excelsior Care Group is seeking an experienced RN/LPN MDS Coordinator to oversee MDS and care planning services across two facilities, Lake Worth and Boynton Beach, FL. The role requires travel between Palm Beach Nursing and Heartland Nursing and Rehabilitation Center locations as needed.

The ideal candidate has strong MDS/RAI knowledge, excellent organizational and communication skills, and a commitment to regulatory compliance and quality resident care.

Qualifications

• Must hold a current Florida RN or LPN license.
• MDS experience strongly preferred.
• Minimum 2 years LTC/skilled nursing experience.
• Strong knowledge of RAI/MDS regulations and reimbursement processes.
• Excellent organizational and communication skills.

Responsibilities

• Coordinate and oversee completion and submission of MDS assessments in compliance with federal and state regulations.
• Ensure MDS assessments and supporting documentation are accurate, complete, and submitted timely.
• Coordinate interdisciplinary care plan meetings and ensure individualized, resident-centered care plans.
• Monitor Quality Measures (QM) and Quality Indicators (QI) and assist with corrective action plans.
• Maintain accurate MDS, RAI, and care planning documentation.
• Collaborate with nursing, therapy, dietary, social services, physicians, and other interdisciplinary team members.
• Assist with discharge planning and coordination of resident services.
• Participate in facility surveys, audits, and regulatory inspections.
• Support quality assurance and performance improvement initiatives related to MDS and care planning.
