Job summary

Palm Beach Nursing and Heartland Nursing and Rehabilitation Center is seeking an experienced RN/LPN MDS Coordinator to oversee MDS and care planning across both facilities. You will travel between Lake Worth and Boynton Beach locations to coordinate MDS submissions and ensure accurate documentation.

The ideal candidate has strong MDS/RAI knowledge, excellent organizational and communication skills, and a commitment to resident-centered care, regulatory compliance, and high-quality outcomes.

Qualifications

• Active Florida RN or LPN license.
• MDS experience preferred.
• At least 2 years in long-term care.
• Strong knowledge of RAI/MDS regulations and reimbursement.
• Excellent organizational, communication, and time-management skills.
• Ability to manage two facilities.
• Reliable transportation for travel between locations.
• Resident-centered, compassionate approach.

Responsibilities

• Coordinate and oversee completion and submission of MDS assessments in compliance with regulations.
• Ensure MDS assessments and documentation are accurate and timely.
• Coordinate interdisciplinary care plan meetings for individualized resident care plans.
• Monitor Quality Measures and Quality Indicators and assist with corrective actions.
• Maintain MDS, RAI, and care planning documentation.
• Collaborate with nursing, therapy, dietary, social services, physicians, and the team.
• Assist with discharge planning and coordination of resident services.
• Participate in surveys, audits, and regulatory inspections.
• Support quality assurance and performance improvement for MDS and care planning.
• Evaluate residents’ needs to ensure appropriate interventions.
• Communicate effectively with residents, families, and the team.
• Travel between Lake Worth and Boynton Beach facilities to provide MDS coordination.

Locations: Lake Worth, FL & Boynton Beach, FL
Schedule: Full-Time
Compensation: $38.00–$46.00 per hour

Palm Beach Nursing and Heartland Nursing and Rehabilitation Center are seeking an experienced RN/LPN MDS Coordinator to oversee MDS and care planning services across both facilities. This position will require the ability to travel between our Lake Worth and Boynton Beach locations as needed.

The ideal candidate has strong MDS/RAI knowledge, excellent organizational skills, and a commitment to ensuring accurate assessments, regulatory compliance, and quality resident care.

Key Responsibilities

• Coordinate and oversee the completion and submission of MDS assessments in compliance with federal and state regulations.

• Ensure MDS assessments and supporting documentation are accurate, complete, and submitted timely.

• Coordinate interdisciplinary care plan meetings and ensure individualized, resident-centered care plans.

• Monitor Quality Measures (QM) and Quality Indicators (QI) and assist with corrective action plans.

• Maintain accurate MDS, RAI, and care planning documentation.

• Collaborate with nursing, therapy, dietary, social services, physicians, and other interdisciplinary team members.

• Assist with discharge planning and coordination of resident services.

• Participate in facility surveys, audits, and regulatory inspections.

• Travel between the Lake Worth and Boynton Beach facilities to provide MDS coordination and support.

Qualifications

• Current Florida RN or LPN license.

• Previous MDS experience strongly preferred.

• At least 2 years of long-term care or skilled nursing experience preferred.

• Strong knowledge of RAI/MDS regulations, assessments, and reimbursement processes.

• Excellent organizational, communication, and time-management skills.

• Ability to manage responsibilities across two facilities.

• Reliable transportation and ability to travel between locations.
