Job summary

Palm Beach Nursing and Heartland Nursing and Rehabilitation Center is seeking an experienced RN/LPN MDS Coordinator to oversee MDS and care planning services across both Lake Worth and Boynton Beach facilities. You will travel between locations as needed to coordinate assessments and resident care.

The ideal candidate has strong MDS/RAI knowledge, excellent organizational and communication skills, and a commitment to regulatory compliance and quality resident care.

Qualifications

• Florida RN or LPN license required.
• MDS/RAI knowledge and experience preferred.
• 2+ years in long-term care or skilled nursing.
• Strong knowledge of RAI/MDS regulations and reimbursement processes.
• Excellent organizational, communication, and time-management skills.

Responsibilities

• Coordinate completion and submission of MDS assessments per regulations.
• Ensure accuracy and timeliness of MDS and supporting docs.
• Lead interdisciplinary care plan meetings for resident-centered plans.
• Monitor QM/QI and assist with corrective action plans.
• Maintain MDS, RAI, and care planning documentation.
• Collaborate with nursing, therapy, dietary, social services, physicians, and team members.
• Assist with discharge planning and coordination of resident services.
• Participate in surveys, audits, and inspections.
• Support quality assurance initiatives related to MDS and care planning.
• Evaluate residents’ needs to determine appropriate interventions.
• Communicate effectively with residents, families, and the team.
• Travel between Lake Worth and Boynton Beach facilities for coordination.

Facilities: Palm Beach Nursing & Heartland Nursing and Rehabilitation Center
Locations: Lake Worth, FL & Boynton Beach, FL
Schedule: Full-Time
Compensation: $38.00–$46.00 per hour

Palm Beach Nursing and Heartland Nursing and Rehabilitation Center are seeking an experienced RN/LPN MDS Coordinator to oversee MDS and care planning services across both facilities. This position will require the ability to travel between our Lake Worth and Boynton Beach locations as needed.

The ideal candidate has strong MDS/RAI knowledge, excellent organizational skills, and a commitment to ensuring accurate assessments, regulatory compliance, and quality resident care.

Key Responsibilities

• Coordinate and oversee the completion and submission of MDS assessments in compliance with federal and state regulations.

• Ensure MDS assessments and supporting documentation are accurate, complete, and submitted timely.

• Coordinate interdisciplinary care plan meetings and ensure individualized, resident-centered care plans.

• Monitor Quality Measures (QM) and Quality Indicators (QI) and assist with corrective action plans.

• Maintain accurate MDS, RAI, and care planning documentation.

• Collaborate with nursing, therapy, dietary, social services, physicians, and other interdisciplinary team members.

• Assist with discharge planning and coordination of resident services.

• Participate in facility surveys, audits, and regulatory inspections.

• Support quality assurance and performance improvement initiatives related to MDS and care planning.

• Evaluate residents’ strengths, needs, and limitations to ensure appropriate care interventions.

• Communicate effectively with residents, families, physicians, and the interdisciplinary team.

• Travel between the Lake Worth and Boynton Beach facilities to provide MDS coordination and support.

Qualifications

• Current Florida RN or LPN license.

• Previous MDS experience strongly preferred.

• At least 2 years of long-term care or skilled nursing experience preferred.

• Strong knowledge of RAI/MDS regulations, assessments, and reimbursement processes.

• Excellent organizational, communication, and time-management skills.

• Ability to manage responsibilities across two facilities.

• Reliable transportation and ability to travel between locations.
