Job summary

CareSource is seeking a Community Based Care Coordinator, Duals Integrated Care, to manage and coordinate care for dual-eligible beneficiaries across diverse community settings. You will collaborate with healthcare providers, resources, and interdisciplinary teams to develop individualized care plans and ensure smooth transitions of care.

The role requires current licensure as RN, SW, or Clinical Counselor, a health care degree, willingness to travel up to 50%, and vaccination compliance.

Qualifications

• Current, unrestricted clinical license in RN, Social Worker or Clinical Counselor.
• Bachelor's degree in a health care field required; equivalent years of relevant experience may be accepted.
• Prior experience in care coordination or working with dual-eligible populations preferred.

Responsibilities

• Engage with members in community settings to establish care coordination relationships.
• Liaise between providers, resources, and dual-eligible beneficiaries for seamless care.
• Conduct comprehensive assessments of physical, mental, and social needs.
• Develop and implement individualized care plans addressing medical and social needs.
• Coordinate with ICT to create holistic care plans and monitor adherence.
