CareSource is seeking a Community Based Care Coordinator, Duals Integrated Care to manage and coordinate care for dual-eligible beneficiaries. This role blends health services with community resources to improve outcomes and quality of life for individuals with complex health needs.

You will engage with providers, conduct comprehensive assessments, and develop individualized care plans with an interdisciplinary team.

Qualifikationen

• Intermediate proficiency with Microsoft Office (Outlook, Word, Excel).
• Understanding of Medicare and Medicaid programs and community resources.
• Strong interpersonal and communication skills to engage members, families, and providers.

Aufgaben

• Engage with members in community settings to establish care coordination relationships.
• Liaise between healthcare providers, community resources, and beneficiaries to ensure care transitions.
• Conduct comprehensive assessments of physical, mental, and social needs.
• Develop and implement individualized care plans addressing medical and social requirements.
• Collaborate with the interdisciplinary care team to create holistic plans.
• Assist members in accessing housing, transportation, food assistance, and services.
• Educate members about benefits under Medicare and Medicaid.
• Provide education on chronic condition management and preventive care.
• Promote healthy lifestyle choices and self-management.
• Monitor health status and adjust plans; follow up after hospitalizations.
• Coordinate with physicians and other providers to share information.
• Participate in care team meetings and document interactions and outcomes.
• Collect and analyze data to evaluate effectiveness and improve care.
• Advocate for dual-eligible beneficiaries within the healthcare system.
• Empower members to participate in their healthcare decisions.
• Report abuse or neglect as required by state law; on-call as assigned.
• Travel to member/provider/community visits as needed and per program requirements.
