Seven Hills Foundation is seeking a Direct Support Professional III to provide one-to-one support in a private home in Waltham, MA. You will help individuals with disabilities build independence, engage in meaningful activities, and maintain a safe, supportive environment.

Responsibilities include implementing Individual Support Plans, assisting with life skills and community activities, coordinating medical appointments, and documenting services with confidentiality.

Qualifications

• Person-centered care experience with individuals who have disabilities.
• Ability to document services and maintain confidentiality.
• Strong teamwork and family/caregiver collaboration.

Responsibilities

• Provide person-centered support in a private home and community.
• Implement Individual Support Plans, behavior supports, and therapeutic routines.
• Assist with life skills, household responsibilities, and community activities.
• Coordinate medical appointments and support overall health and wellness.
• Document services and maintain confidential records in accordance with agency standards.
