Job summary

Seven Hills Foundation is seeking a Direct Support Professional to provide one-to-one support in a private home setting. You will help individuals with disabilities build independence, engage in meaningful activities, and maintain a safe, supportive living environment in partnership with their caregivers.

Schedule includes weekday and weekend shifts with pay of $25/hour on weekdays and $27/hour on weekends.

Responsibilities

• Provide person-centered support in a private home and the community
• Implement Individual Support Plans, behavior supports, and therapeutic routines
• Assist with life skills, household responsibilities, and community activities
• Coordinate medical appointments and support overall health and wellness
• Collaborate with families, caregivers, clinicians, and team members
• Document services and maintain confidential records in accordance with agency standards
