Qualifications

• High school diploma/ GED/ HiSET required.
• 1 – 3 years of experience in direct support, disability services, or related areas.

Responsibilities

• Provide person-centered support in a private home and the community.
• Implement Individual Support Plans, behavior supports, and therapeutic routines.
• Assist with life skills, household responsibilities, and community activities.
• Coordinate medical appointments and support overall health and wellness.
• Collaborate with families, caregivers, clinicians, and team members.
• Document services and maintain confidential records in accordance with agency standards.
