Seven Hills Foundation is seeking a Direct Support Professional to provide one-to-one support in a private home in Southborough, MA. The role focuses on helping individuals with disabilities build independence, engage in meaningful activities, and maintain a safe, supportive environment.

• Provide person-centered support in a private home and the community.

• Assist with life skills, household responsibilities, and community activities.
• Coordinate medical appointments and support overall health and wellness.
• Collaborate with families, caregivers, clinicians, and team members.
• Document services and maintain confidential records in accordance with agency standards.
