Learning Services, A Collage Rehabilitation Partner in Riverton, UT, seeks a compassionate Direct Support Professional (DSP) to support adults with brain injuries. You will implement rehabilitation strategies, assist with daily living activities, and encourage progress toward personal independence in a home-like setting.

Responsibilities

• Promote independence by assisting with activities of daily living (ADLs), community living skills, and rehab goals.
• Assist with medication administration, monitor nutrition plans, and prepare balanced meals daily.
• Provide proactive behavioral support and implement behavior management plans while maintaining professional boundaries and mutual trust.
• Identify changes in clients' behavioral, emotional, cognitive, or physical conditions and alert the treatment team to ensure timely care.
• Safely transport clients to medical appointments, group outings, errands, and recreational activities.
• Complete shift documentation including daily data sheets, goal sheets, vital/seizure flow sheets, supervision checklists, and incident reports in line with CARF, state, and federal standards.
• Maintain a clean, organized, safe living environment in alignment with licensing requirements.
• Participate in shift hand-offs and coordinate with family members, care teams, and leadership.
