Job summary

Towne Home Care in Monmouth County, NJ seeks a Direct Support Professional to provide one-on-one supports in home and community settings under ISP guidance.

You will assist with personal care, domestic tasks, mobility, and accompany clients to outings, while documenting progress and communicating with the interdisciplinary team. Candidates with DSP experience, CPR/BLS, and reliable transportation are encouraged to apply; daytime shifts are strongly preferred.

Qualifications

• Proven DSP experience preferred.
• Smartphone used for case info and clocking in/out.
• Knowledge of basic nutrition and personal hygiene standards.
• Caring and compassionate personality.
• Ability to assess changing situations and respond accordingly.

Responsibilities

• Provide one-on-one supports to individuals in private homes and in the community per ISP.
• Act as a member of the interdisciplinary team to assist with ISP development.
• Assist and support the individual with domestic and household tasks.
• Assist individuals with personal care activities.
• Assist individuals with mobility and physical therapies/exercises.
• Provide companionship and basic emotional or psychological support.
• Complete logs linking activities to ISP goals.
