Job summary

Chapters Health System is seeking a capable supervisor to manage the interdisciplinary care team in a Florida-based setting. You will coordinate care, monitor adherence to care plans, and guide staff through established workflows and regulatory requirements.

The role emphasizes leadership, staff development, and participation in QAPI initiatives to improve patient outcomes and safety. A bachelor's degree and healthcare experience are preferred.

Qualifications

  • Supervisory responsibilities and staff coaching are required.
  • Experience with regulatory guidelines and Medicare Conditions of Participation (CoPs).
  • Leadership in interdisciplinary teams and service improvement.

Responsibilities

  • Carry out supervisory responsibilities per Leadership Guidelines.
  • Plan, assign, and direct work; evaluate performance and provide coaching.
  • Coordinate patient care with the IDT and external partners.
  • Ensure workflows and procedures are followed; monitor care plans.
  • Lead QAPI, utilization review, and risk management activities.
  • Support staffing, scheduling, and resource stewardship.

Education And Experience

Bachelors degree preferred. Hospice or home health experience required. Management experience preferred.

Certificates, Licenses, Registrations

Must possess a valid Florida drivers' license, current auto insurance and reliable transportation. Licensure in Nursing, Social Work or certification in field preferred.

Mathematical Skills

Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals. Ability to compute rate, ratio, and percent and to draw and interpret bar graphs.

Certification of completion of Alzheimer's Disease and Related Dementias Training through the Florida Department of Elder Affairs.

Active BLS for healthcare professionals from the American Heart Association or Red Cross. Employees hired prior to 12/31/2022 must obtain certificate prior to 3/31/2023.

Reasoning Ability

Ability to define problems, collect data, establish facts, and draw valid conclusions. Ability to interpret an extensive variety of technical instructions in mathematical or diagram form and deal with several abstract and concrete variables. Ability to communicate, concentrate and document effectively and accurately.

Language Skills

Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations. Ability to write reports, business correspondence, and procedure manuals. Ability to effectively present information and respond to questions from groups of managers, clients, customers, and the general public.

Position Requirements (not All Inclusive)

Carry out supervisory responsibilities in accordance with Hope’s Leadership Guidelines. Conduct training of employees; planning, assigning, and directing work; appraising performance; rewarding, coaching, and guiding employees; addressing complaints and resolving work‑related issues successfully. Provide stewardship of organization resources.

Communicate successfully with peers, supervisors, public, and staff members in a manner that represents Hope’s culture of a collaborative, positive work environment.

Supervise the patient care activities of the interdisciplinary team: coordinate and monitor the implementation of the care plan.

Assure the utilization of workflows, guidelines, and procedures.

Monitor documentation and implementation of physical, psychosocial, emotional, and spiritual interventions for the patient/family plan of care.

Collaborate, facilitate, and communicate with the IDT, and all internal and external customers.

Provide effective decision‑making and problem‑solving of intra‑and inter‑team activities.

Actively participate and lead QAPI, utilization review, and risk management activities to improve outcome measures, satisfaction, and patient safety. Monitor, utilize and disseminate team’s data.

Provide appropriate input for yearly and ongoing employee evaluation by evaluating quality of care delivered by all staff and staff participation in organization activities. Perform supervisory visits with team members.

Assure adequate staffing and staffing ratios through team scheduling and communication. Monitor productivity of assigned staff. Provide time management oversight and guidance for the team.

Assist in the efficient and cost‑effective management of team. Provide direction on the stewardship of drugs, biologicals, supplies, and staff resources.

Provide leadership and formal and/or informal education to team, organization staff and the community through inservice presentations, team meetings, reports, case conferences or small group discussion.

Provide consultation and/or coordinated patient/family visits with team staff for complex cases.

Participate in service recovery, collaborating with Managers, Directors, and Chiefs as appropriate.

Adhere to regulatory guidelines and Medicare Conditions of Participation (CoPs).

Maintain essential relationships with assigned nursing facilities, assisted living facilities, and hospitals.

Facilitate effective and safe transition of patients between care settings.

Compensation Pay Range

$24.30 - $36.16
