Job summary

Catholic Charities of the Archdiocese of Chicago is seeking a Case Manager - Domestic Violence to perform comprehensive client assessments, develop and coordinate individualized service plans, and provide advocacy and resource support.

You will maintain documentation, collaborate with care teams, and promote continuous learning to improve outcomes, with essential functions including mobility tasks and compliance checks.

Qualifications

• Bachelor's degree in a related field and 3–5 years of relevant experience.
• Excellent written and verbal communication skills.
• Strong interpersonal skills.
• Ability to work independently and as part of a team.

Responsibilities

• Conduct comprehensive assessments to determine client needs and service eligibility, and develop individualized service plans.
• Provide direct support and guidance, linking clients to housing, employment training, financial aid, and counseling.
• Maintain accurate documentation of client interactions and progress per CCAC policies; advocate for clients in the community.
• Collaborate with multidisciplinary teams to coordinate services and engage in outreach to raise awareness.
• Engage in ongoing learning and training to improve performance and contribute to team success.
