Job summary

WESTCARE CALIFORNIA INC is seeking a Lead Diversion Specialist focused on reporting, data quality, and referral oversight. You will manage end-to-end referrals, assign caseloads, and ensure accurate data and timely updates to Fresno County.

You will train staff and coordinate with multiple teams to secure housing and community services for clients. The role emphasizes data integrity, impact measurement, and seamless collaboration with the Program Director and FCCoC partners to improve diversion

Qualifications

• Two years of case management or counseling experience.
• Two-year degree preferred in Psychology or Social Services.
• High School diploma or equivalent required.

Responsibilities

• Manage end-to-end referral process for diversion programs and assign caseloads.
• Ensure timely and accurate processing of referrals with a smooth workflow.
• Generate weekly and ad-hoc reports to assess program effectiveness.
• Provide training to new Diversion Specialists and staff on data entry and reporting.
• Oversee HMIS files quality assurance and conduct data quality audits.
• Coordinate with FSC Case Management and crisis housing providers for integrated care.
• Assist clients with housing search, transportation, and financial assistance.
• Meet with clients weekly to support progress toward individualized service plans.
