Job summary

Catholic Charities Twin Cities is seeking a compassionate Diversion Navigator (On Call/Casual) in Minneapolis to assist individuals and families seeking emergency shelter. You will assess needs, provide trauma-informed guidance, and help identify safe alternatives to shelter while connecting clients to community resources.

Responsibilities include crisis screening, developing diversion plans, coordinating with partners, and documenting outcomes in HMIS.

Qualifications

• Post-secondary education required.
• 2 years of relevant experience in human services, housing, or related fields.
• Knowledge of housing programs and city regulations, including landlord/tenant law.
• Experience in intake, assessment, or eligibility determination processes.
• Experience in landlord/tenant mediation or housing navigation.
• Working knowledge of community resources for homelessness or housing instability.
• Excellent computer skills including HMIS and Microsoft Office.

Responsibilities

• Conduct initial screenings, crisis stabilization, and diversion assessments for callers seeking emergency shelter.
• Collaborate with clients to develop diversion plans including mediation, short-term financial assistance, or linkage to housing resources.
• Maintain relationships with community partners to connect clients to appropriate services.
• Coordinate with County Financial Assistance workers to verify eligibility and process payments.
• Track and report data in HMIS and other systems accurately.
• Maintain a resource referral database and stay informed about housing stabilization initiatives.
• Provide education, coaching, and advocacy to support housing stability.
• Contribute to program evaluation and team meetings.
