Job summary

Community House Mental Health Agency in Seattle seeks a dedicated Diversion Case Manager to coordinate outpatient behavioral health services within the assigned diversion program. You will lead treatment planning, document progress notes, and collaborate with the care team to meet client needs and program contracts.

You will work Monday–Friday, 9:00am–4:00pm, with trauma-informed practices, crisis intervention when needed, and a focus on high-quality, confidentiality-respecting care in King

Qualifications

• Coordinate treatment based on client needs and the treatment plan.
• Maintain timely progress notes and documentation in EMR.
• Collaborate with care team to meet program goals.

Responsibilities

• Serve as Primary Case Manager for assigned caseload, coordinating client needs and services.
• Develop individualized plans and ensure timely intake into the program.
• Collaborate with care team to ensure access to treatment and appointments.
• Provide crisis intervention and coordinate with external providers when needed.
• Maintain confidentiality and adhere to trauma-informed care principles.
