Job summary

Salinas Valley Health is seeking a Disease-Specific Care Coordinator to oversee the hospital’s disease-specific program and act as a clinical resource for staff and leadership. You will drive quality improvement, data reporting, and certification activities.

The ideal candidate has a Bachelor’s Degree in Nursing, California RN licensure, at least four years of clinical nursing experience (neuroscience preferred), and working knowledge of regulatory requirements.

Qualifications

• Bachelor’s Degree in Nursing preferred.
• Licensed as a Registered Nurse in California.
• Clinical nursing experience with 4+ years; neuroscience preferred.
• Knowledge of TJC Disease-Specific and regulatory requirements.
• Experience with performance improvement and data reporting.

Responsibilities

• Coordinate and oversee the disease-specific program.
• Lead data collection, analysis and reporting for quality and compliance.
• Facilitate performance improvement and certification activities.
• Mentor staff and ensure policy adherence and education.
• Serve as liaison between nursing, medical staff and leadership.
