HCT seeks an experienced nurse professional to serve as the Interim Disease Specific Care Coordinator for a 250+ bed hospital in California. Responsibilities include:

• Leading and coordinating disease-specific certification programs, driving quality improvement initiatives, ensuring regulatory compliance, and serving as a clinical resource for physicians, nursing staff, and multidisciplinary teams.
• The role focuses on improving patient outcomes through evidence-based practice, education, data analysis, and performance improvement while maintaining compliance with Joint Commission and other disease-specific certification standards.

Requirements:

• Education: Bachelor's Degree in Nursing preferred.
• Experience: Previous program development and implementation experience preferred with a minimum of four (4) years of clinical nursing experience; recent experience in acute clinical neurosciences preferred; current knowledge of hospital regulations, patient rights and consent law; thorough knowledge of TJC Disease-Specific and other regulatory requirements.
• Certifications: Clinical Nurse Specialist/Nurse Practitioner certification preferred; BLS.
• EMR: Epic experience required.

Compensation:
