Job summary

UVA Health is seeking a Discharge Transportation Coordinator to manage patient transport across the health system from UVA Health Town Center One. You will coordinate ambulances, wheelchair vans, taxis, and community options, balancing patient needs, insurance, and availability to ensure smooth movements.

You’ll work with nurses, case managers, social workers, vendors, and community partners to resolve issues and ensure timely discharges.

Responsibilities

• Coordinate patient discharge transportation using ambulances, wheelchair vans, taxis, buses, and other resources.
• Communicate with patients, families, clinicians, and vendors to arrange transport solutions.
• Navigate insurance coverage and eligibility guidelines to support discharge planning.
• Prioritize requests while managing multiple cases in a dynamic healthcare environment.
• Investigate transportation issues and support positive patient experiences.
• Track activity, process invoices, and maintain records per procedures.
• Support process improvements and onboarding of new team members.
