Qualifications

• Bachelor’s degree in social work, health and human services, or allied health field or equivalent experience (4 years).
• BL S certified, or ability to obtain within 6 months of employment.
• Must demonstrate effective communication and customer service skills, human relation skills and time management skills.

• Assist in communicating the discharge plan to patients and families.
• Facilitate safe and timely transitions of care from acute care to post-acute settings.
