Job summary

Rimrock Foundation seeks a Discharge Planner to provide therapeutic supervision and coordinate services for adults in inpatient treatment. You will ensure safety and security of the facility, adhere to HIPAA and 42 CFR Part 2, and support counselors as needed.

The role requires organizing discharge plans, arranging continuing care, and collaborating with the therapeutic team to arrange housing, legal, and financial supports. Strong communication and confidentiality are essential.

Qualifications

• Bachelor’s degree in a Human Services or Social Work field or equivalent experience required.
• At least 1 year of experience in Mental Health or SUD preferred.

Responsibilities

• Provide discharge planning and arrange continuing care, psychiatric monitoring as needed, and medication monitoring for all discharging clients.
• Coordinate care: schedule client appointments and coordinate legal, financial and housing services as assigned.
• Provide therapeutic supervision of clients and maintain the client’s program schedule.
• Report incidents and emergencies per Rimrock policies.
• Assist clients with therapeutic assignments.
• Attend staff meetings and in-services.
• Provide orientation to new clients.
• Other duties as assigned.
