Job summary

Ridgeview Institute Monroe is seeking a Discharge Planner – Social Services to coordinate discharge plans, link patients with home and community resources, and support families in navigating treatment options. The role involves crisis intervention, short- and long-term family therapy, and collaboration with care teams to ensure seamless transitions.

Qualified candidates typically hold a Bachelor’s in Social Work or related field, with at least 1 year hospital experience preferred.

Qualifications

• Bachelor's Degree in Social Work or clinical related mental health field preferred.
• Minimum of one year experience in related disciplines with hospital experience preferred.
• CPI training within 30 days of employment and competency exam for the area on hire.

Responsibilities

• Communicate with referral source within the first 24 hours of admission.
• Identify outside professionals involved with the patient for discharge planning.
• Oversee ongoing communication of treatment progress and discharge timing to outside parties.
• Assist with discharge planning, scheduling of appointments, and parent/family communication.
