Rome Health seeks a Discharge Planning Referral Specialist to support the Continuum of Care team, including directors, case managers, social workers and clinicians, in meeting discharge planning and continuity of care needs for patients and caregivers.

The role focuses on advancing patient referrals, coordinating with referral sources, and assisting care managers through intake using the Care Portal to manage skilled nursing facility and home care referrals.

Qualificações

• Knowledge of Case Management and Social Worker principles.
• Excellent written and verbal communication and strong organizational skills.
• Ability to prioritize and follow oral and written instructions.
