• Comparable combination of education and experience may substitute for the above requirements.

Responsibilities

• Performs discharge planning assessments from admission, evaluating baseline function vs. discharge needs.
• Collaborates with physicians, nurses, care management, therapy services to facilitate transitions of care.
• Documents assessments, discharge plans, referrals, interventions, and actions in the medical record.
