Qualifications

• Experience in social work case management for mentally ill populations.

Responsibilities

• Coordinate discharge planning for patients with Severe and Persistent Mental Illness.
• Develop relationships with community agencies for financial assistance.
• Identify patients' release-planning needs and provide support.
• Coordinate medication and make appointments for continued care.
• Maintain documentation of services provided.
