Job summary

Woodside Rehab & Nursing, part of Vierra Communities, seeks a Social Services and Discharge Planning Assistant in Silver Spring, MD. The role supports development, implementation, supervision, and evaluation of the Social Services Department to help residents achieve their highest practicable well-being; you will coordinate medically related social services according to regulations.

Requirements include a bachelor's degree in social work or related field, at least one year supervised experience

Qualifications

Bachelor's degree in social work or a related field.
Minimum 1 year supervised social work experience in a health care setting.
Licensed social worker required.

Responsibilities

Oversee development, implementation, supervision and evaluation of the Social Services Department to meet residents' needs.
Coordinate transitions and discharge planning in collaboration with the interdisciplinary team.
Identify and arrange community-based services and follow-up care as needed.
