Qualifications

• High School Diploma is required.
• Junior College/Technical School preferred.
• BLS certification is required.
• Working knowledge of medical terminology.
• Excellent verbal and written communication with patients and staff.

Responsibilities

• Assist Care Facilitator and Social Worker with discharge planning.
• Coordinate discharge services such as home health, DME, rehab placement.
