Job summary

St. Mary's Medical Center seeks a full-time Discharge Planning Specialist to support the Care Facilitator and/or Social Worker in coordinating safe, timely discharges.

Responsibilities include arranging post-acute services such as home health, DME, rehab placement, and connecting patients with community resources while maintaining excellent communication with patients, families, physicians, and hospital staff.

Qualifications

• BLS required; working knowledge of medical terminology.
• Excellent verbal and written communication with patients, families, physicians, and hospital staff.
• High school diploma required; junior college/technical school preferred.
• Education and training in hospital healthcare transitional care and discharge planning preferred.

Responsibilities

• Assist the Care Facilitator and/or Social Worker with discharge planning to promote timely, safe discharge.
• Coordinate discharge services such as home health, DME, Rehab placement, and community resources.
