Job summary

Qualifications

• High School Diploma required; Junior College/Technical School preferred.
• Education and training in hospital healthcare transitional care and discharge planning preferred.
• Excellent verbal and written communication with patients, families, physicians, and hospital staff.

Responsibilities

• Assist the Care Facilitator and/or Social Worker with discharge planning responsibilities to promote timely and safe discharge services.
• Coordinate discharge services such as home health, DME, Rehab placement, and other community resources.
• Under supervision, facilitate and coordinate discharge planning across hospital departments.
