Job summary

St. Mary's Medical Center seeks a full-time Discharge Planning Specialist to support the Care Facilitator and/or Social Worker in coordinating safe, timely discharges.

Responsibilities include arranging post-acute services such as home health, DME, rehab placement, and connecting patients with community resources while maintaining excellent communication with patients, families, physicians, and hospital staff.

Qualifications

• BLS required; working knowledge of medical terminology.
• Excellent verbal and written communication with patients, families, physicians, and hospital staff.
• High school diploma required; junior college/technical school preferred.
• Education and training in hospital healthcare transitional care and discharge planning preferred.

Under the supervision of the Care Facilitator and or Social Worker, the Discharge Planning Specialist facilitates and coordinates discharge services such as home health, DME, Rehab placement, and other community resources or agencies.

Education Requirements:

High School Diploma is required. Junior College/Technical School preferred.

Experience:

Education and training in hospital healthcare transitional care, and discharge planning beyond the hospital setting, preferred.

Certifications/Skills:

BLS required. Working knowledge of medical terminology. Excellent communication skills-verbal and written ability to work with patients, families, physicians, and other hospital departments.
