Appalachian Community Services in Waynesville, NC seeks a Discharge Planner for the Adult Recovery Unit to coordinate discharge planning for adults 18–64 challenging mental health and substance use issues. The role focuses on stabilizing clients and preparing for community-based care after discharge.

Responsibilities include assessing needs, arranging post-discharge services, educating clients and families, and documenting plans in Credible within a collaborative treatment team.

Qualifications

• High school diploma or equivalent required.
• One year experience working with populations served preferred.
• Knowledge of Recovery concepts and ability to assist RNs and appropriate persons to convey this knowledge to clients.

Responsibilities

• Assess client needs and develop individualized discharge plans.
• Coordinate follow-up appointments, referrals, and community services.
• Educate clients and families on available recovery resources and support options.
• Complete timely and accurate documentation while collaborating with the treatment team.
• Collaborate with therapists, nurses, CMHAs, and prescribers to ensure discharge plans address all areas of the client’s recovery.
