Qualifications

• Comprehensive age-specific psychosocial assessment and development of treatment plan.
• Ability to formulate treatment goals and refer to other disciplines as needed.
• Provides counseling to patients and families using diverse techniques.
• Coordinates discharge planning and transition of care with care management.
• DSM multiaxial assessment and referrals to mental health services as needed.
• Maintains documentation and follows department policies.
• Participates in teaching and mentoring activities for staff and interns.

Responsibilities

• Provides psychosocial interventions addressing emotional, financial, environmental, and safety issues.
• Collaborates with physicians, nurses, social workers, and community agencies to ensure continuity of care.
• Assists with guardianship or commitment matters when appropriate within legal and ethical guidelines.
• Develops and documents post-hospital care plans and coordinates transitions of care.
• Supports quality initiatives and participates in ongoing education and professional development.
