Job summary

Baystate Health, a leading health system in western Massachusetts, is seeking an Inpatient Social Worker for Baystate Medical Center in Springfield. The role supports psychosocial assessment, counseling, and discharge planning for hospitalized patients on a full-time schedule with rotating weekends and holidays.

Qualifications

• Master's Degree in Social Work required.
• Minimum of three years as a practicing social worker in a health care or social service setting preferred.
• LICSW or LCSW with Master's degree considered.

Responsibilities

• Conduct comprehensive, age-specific psychosocial assessments and develop treatment plans.
• Incorporate cultural and spiritual factors into assessments and care planning.
• Assess and document risk of abuse/neglect for vulnerable populations as appropriate.
• Provide counseling services to patients and families and facilitate effective coping with medical issues.
• Coordinate discharge planning and transitions of care to ensure timely, appropriate post-hospital services.
