Job summary

Baystate Health is seeking a licensed social worker to assess and address the psycho-social needs of patients and their families, coordinating discharge planning and referrals to post-hospital care. You will provide counseling and crisis intervention, and collaborate with staff and community providers to ensure smooth transitions.

Required is a bachelor’s degree from a CSWE-accredited program, two years of related experience, and an MA license at the LSW level.

Qualifications

• Bachelor’s degree from a CSWE-accredited school.
• Two years of related professional experience required.
• Valid social work license from the Commonwealth of Massachusetts, at least the LSW level.

Responsibilities

• Assess and address psycho-social needs of patients and families.
• Identify patients who will require discharge planning services.
• Provide counseling, crisis intervention and referrals to post-hospital care.
• Consult with staff and coordinate with community-based providers as required.
