Job summary

Mary Washington Healthcare is seeking an experienced Case Manager, Discharge (MSW) to organize and expedite discharge plans for medically complex cases. The role emphasizes collaboration with clinical partners, patient education, and guardianship/end-of-life decision support.

The MSW will lead care coordination, mentor junior staff, and ensure timely communication of discharge needs, while addressing barriers to discharge and post-discharge follow-up.

Qualifications

• Bachelor's degree in Social Work required.
• Minimum of one year experience in social work.
• MSW required with LMSW eligibility or certification preferred.
• Experience with Windows-based programs, email, and Microsoft Word is preferred.

Responsibilities

• Coordinate care and discharge planning for medically complex patients.
• Serve as a resource for the care team and mentor associates and interns.
• Assess patient needs and communicate discharge plans with families and the care team.
• Provide information and education on community resources and guardianship/end-of-life decisions.
