Mary Washington Healthcare is seeking a Case Manager, Discharge (MSW) to organize and expedite a treatment plan of care for medically complex social issues related to hospital progression of care. You will identify discharge needs, develop a discharge plan, and educate families about community resources.

The ideal candidate holds a Master’s degree in Social Work, has at least one year of social work experience or two-semester internships in care coordination, and may pursue LMSW.

Qualifications

• Master's degree in Social Work required.
• Minimum one year of social work experience or two-semester internship in Care Coordination.
• Experience with Windows-based programs, e-mail, and Microsoft Word required.
• Healthcare experience preferred; Case Management Certification strongly desired.

Responsabilités

• Coordinates care with clinical partners; helps patients advance towards outcomes.
• Serves as a mentor for new social work Associates and interns.
• Assesses long-term/future patient care needs.
• Communicates treatment plan updates with care partners.
• Develops, coordinates, and communicates discharge plans for medically complex cases.
• Assists with audits and assessments of discharge planning documentation.
• Addresses concerns related to delays in discharge and barriers to discharge.
• Provides post-discharge support and education to patients and families.
• Conducts psychosocial assessments as needed for discharge planning.
• Participates in quality improvement activities.
