Job summary

Qualifications

• Bachelor’s Degree in Social Work from an accredited school.
• L.S.W. or MSW license in Ohio; Michigan-BSW accepted.
• 3–5 years inpatient psychiatric unit experience.
• Knowledge of community resources and discharge planning.

Responsibilities

• Complete initial assessment and discharge plan within 24–48 hours with family and medical staff.
• Participate in multidisciplinary treatment team meetings and gather data for discharge planning.
• Initiate discharge placement options and educate families on community resources.
• Provide individual and group counseling; assess needs including finances for referrals.
• Assess suicidal/homicidal risk and implement safety procedures per policy.
• Document interactions accurately and communicate with relevant departments.
• Stay current with social work standards and provide staff leadership and consultation.
• Advocate for patients’ rights and promote continuity of care.
