Job summary

Kaiser Permanente is seeking a Per Diem RN Case Manager Liaison for inpatient discharge planning on site in Bellevue. This role involves rotating two settings, planning discharges and follow-up care for patients in Kaiser Washington network hospitals, with a variable schedule and some weekend hours.

The position requires current RN license and 3+ years hospital/ambulatory RN experience, plus 2 years in case management and a Case Manager certificate within 24 months.

Qualifications

• Minimum three (3) years of RN medical/surgical/ambulatory clinical experience.
• Minimum two (2) years of RN experience in ambulatory case management or care coordination.
• BA or BS degree.
• Registered Nurse License (Washington) required at hire or Compact License.
• Case Manager Certificate within 24 months of hire.
• Basic Life Support required at hire.

Responsibilities

• Plan discharges and follow-up care for patients with a rotating schedule.
• Collaborate with primary and specialty care teams to ensure adherence to the medical plan.
• Coordinate referrals to home health, long-term care, hospice, and other services.
• Monitor patient progress and advocate for optimal health outcomes.
