Job summary

The University of Miami Health System (UHealth) seeks a dedicated nurse to provide patient-centered care across admission, transfer, and discharge. You will apply the nursing process to anticipate needs, review orders, and collaborate with the interdisciplinary team to ensure safe, timely discharge within daily rounds and handoffs.

Responsibilities include education, medication reconciliation, and coordinating post-discharge follow-up.

Qualifications

• Review orders on admission, transfers, and discharges.
• Communicate patient changes to the team and patient/family within 24–72 hours of admission.
• Assist discharge planning and medication reconciliation.

Responsibilities

• Document discharge teaching and ensure patient/family understanding.
• Facilitate follow-up appointments and schedule required tests/procedures.
• Coordinate with interdisciplinary team to ensure readiness for discharge by 12 noon.
• Serve as liaison between medical/surgical team and patient/family post-discharge.
